Migraines and headaches are considered by the World Health Organization to be among the most disabling diseases, comparable to dementia, paraplegia and active psychosis. They are the second most common reason for incapacity to work. The diseases cause high direct and indirect costs. Incapacity to work due to migraines alone costs 3.1 billion euros per year in Germany, calculated based on 32 million days lost. The risk of depression, anxiety disorders and suicide is 3 to 7 times higher in those affected than in healthy people. The risk of circulatory diseases, heart attack and stroke is 1.5 to 2 times higher than in healthy individuals. This is particularly true for young women under 45 years of age. As part of the COVID-19 pandemic, new forms of headache after infection and vaccination have appeared for the first time.

In the inaugural lecture, data on the clinical phenotypes of headaches after vaccination against COVID-19 with the BNT162b2 mRNA and the ChAdOx1 nCoV-19 vaccine will be presented. New comprehensive analyzes of the headache phenotypes of cluster headaches and headaches during transient ischemic attacks are also presented. The complex trigeminocervical pain-modulating mechanisms for the pathophysiology of migraine are discussed in studies of occipital nerve stimulation. The centrally mediated sensitization to migraines is made clear. The diagnosis and treatment of headache disorders require a sound clinical neurological analysis. This is the key to clarifying specific pathomechanisms and correlates of headache development.
