To plan your admission appointment, we ask you to do the following three things: Your treating doctor will issue a prescription for hospital treatment. Ask your doctor to complete the intake checklist. Fill out the pain calendar and the pain questionnaire (see box on the right). Send all documents and additional copies of all relevant doctor's letters, x-rays, etc. to the address specified on the admission checklist. Depending on the health insurance company, the following regulations apply to the assumption of costs: Numerous health insurance companies have contractually regulated integrated care for their insured persons with our treatment network. For those insured by the AOK Schleswig-Holstein, the Techniker Krankenkasse, the German Employees' Health Insurance Fund, the Hanseatic Health Insurance Fund HEK, the Agricultural Health Insurance Fund Schleswig-Holstein and Hamburg, the Knappschaft Bahn See, the BKK on site and the E.ON company health insurance fund, the admission conditions are met …
